All returns must be accompanied by a RA (Return Authorization) number. Please call our customer service line to obtain a RA. All returns must be accompanied with return form

Returns are subject to restocking fees, to be set at Allstate Medical's discretion. Allstate Medical strives to obtain accurate shipping charges. Shipping rates are calculated at the time of checkout. Items marked Additional Freight Charges May Apply, Allstate Medical reserves the right to charge additional charges. You will be notified prior to shipment if this is necessary. Such charges will be billed separately.
